CARE Services

 

CARE
(Continuous Assistance with Recovery and Empowerment Program)

Individuals with severe and persistent mental illness may need outside support and assistance in the community in order to achieve recovery in their lives. CARE services are provided by a team of case managers who reach out to individuals mainly in their homes or a community setting. Recovery plans and treatment are individualized. Individuals wanting CARE services are able to live any where within the 7 county service area.

Some of the services offered by CARE case managers:

  • Recovery and co-occurring driven case management

  • Medication and symptom management in home and with psychiatrist

  • Person centered and strength based planning

  • Assistance with development of independent living skills

  • Assistance with obtaining and maintaining housing

  • Budgeting and financial development

  • Education related to neurobiological illness

  • Linkage with vocational services and job coaching

  • Crisis intervention assistance

  • Family and support system involvement

  • Community integration

  • Linkage with medical and social services

  • Liaison Services
